We work closely with Maximus Healthcare to ensure that every resident who is discharged and needs to apply for Waiver Services are already eligible and enrolled with the best CHC Choices provider for their needs. Any resident applying for Waiver who was admitted to any facility Saber Healthcare operates will already have the eligibility for LTC MA verified and even IVA's completed saving Maximus the countless resources they typically put toward those initial visit assessments, when the time comes for them to be discharged.
